29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact. A. Clarion Boards; Energy Building; airlock for the fuel conveyance system - On or about 1/2/15, employees were performing servicing and maintenance operations on the airlock for the fuel conveyance system which required the removal of a guard. The machine guard was not reattached to the airlock after lockout devices were removed and prior to re-energizing the airlock system.

Clarion Boards; Energy Building; airlock for the fuel conveyance system - On or about 1/2/15, employees were exposed to amputation hazards from the unguarded rotating airlock components. The fixed guard was not installed on the airlock unit.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. A.) Clarion Boards; 143 Fiberglass Road; Shippenville, PA - On or about 1/3/15 and times prior thereto, the company did not report a work related amputation within 24 hours of it occurring. The amputation occurred on or about 1/2/15 at 3:00 PM. The amputation was not reported to OSHA until 1/5/15 at 3:00 PM.
