29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: a.) Clarion Boards, Inc., 143 Fiberboard Rd, Shippenville, PA, 16254-Storage cabinet next to press enclosure, on or about January 8, 2015, and times prior thereto: Two employee's 3M brand air purifying respirators (#6300 with organic vapor/particulate cartridges), were stored in a storage cabinet adjacent to the Press area of the facility. The respirators were not stored in a way to protect the respirators from items, including but not limited to, damage, contamination, and/or dust.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life: a.) Clarion Boards, Inc., 143 Fiberboard Rd, Shippenville, PA, 16254-Storage cabinet next to press enclosure, on or about January 8, 2015, and times prior thereto: Employees routinely used 3M brand half mask air purifying respirators(#6300) with organic vapor cartridges. A change out schedule for replacing the cartridges had not developed nor incorporated into the employer's respiratory protection program.
