Clarion Boards, Inc. 143 Fiberboard Road, Shippenville, PA: On or about December 4, 2013, and at times prior thereto, employees periodically entered and performed work in the permit required confined spaces located in the facility which included, but were not limited to, the bottom section of the Electrostatic Precipitator. The entry permits utilized by the company did not identify the hazards of the permit required confined space to be entered.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a.) Clarion Boards, Inc. 143 Fiberboard Road, Shippenville, PA: On or about December 4, 2013, and at times prior thereto, employees periodically entered and performed work in the permit required confined spaces located in the facility which included, but were not limited to, the bottom section of the Electrostatic Precipitator. A sign had not been posted on, or around, the bottom section of the Electrostatic Precipitator to identify the area as a permit required confined space.
