OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from the over pressurization of a plastic extruding machine barrel(s): 131 W. Main St., Orange, MA: On or about January 13, 2015, employees were exposed to hazards resulting from the over pressurization of the barrel of a plastic extruding machine. The barrel of a PolySystem extruder machine, model AB-36-6"-SL serial number 90-30150, over pressurized, breaking its retaining plate and shearing (four) one inch diameter cap screws forcibly ejecting the metal parts into the work area occupied by employees.
