29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On the production floor at 131 West Main St, Orange, MA: On June 29, 2023, and at times prior, employees did not utilize the existing energy control procedure to lock out all energy sources when performing cutting blade changes on the J.C. Shuttle machine. On 6/23/2023, an employee received a finger amputation while performing maintenance on the J.C. Shuttle machine. Hi-De Liners Inc was previously cited for a violation of this Occupational Safety and Health standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1441392, citation number 2, item 1, and was affirmed as a final order on January 31, 2020.
