29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i)-(v):  131 West Main Street, Orange, MA: On October 2, 2024, and at times prior, when setting up and threading plastic through the JC Shuttle Machine, which required the machine to be energized for positioning of the plastic, employees were exposed to mechanical hazards. Employees were not removed and safely positioned away from the hazards and were not protected by adequate safeguarding during testing and positioning activities.
