29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 131 West Main St, Orange, MA On 3/17/16 and at times prior, employees did not utilize the existing energy control procedure to lock out all energy sources when setting up the Wolverine treater/printer machine. On 3/11/16, an employee received finger injuries while reaching in to set up an operating treater/printer machine.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. 131 West Main St, Orange, MA On 3/17/16 and at times prior, the energy control procedure for Wolverine treater/printer combination machine did not require the lockout or tagout of the pneumatic source of energy, but did require lockout of the two electrical sources of energy.
