29 CFR 1910.95(d)(3): Repeat employee exposure monitoring was not performed whenever a change in production, process, equipment or controls increases noise exposure:    The employer failed to repeat employees exposure monitoring for noise when process and equipment was changed. A large Indy Plant grinder was added to the plastic grinding room in September, 2013 and a new replacement Husky injection molder was installed in April, 2014. Employees were exposed to the hazard of hearing loss while performing assigned work activity involving the manufacture of plastic containers.   a) A grinder operator was exposed to continuous noise levels of 242 % dose of the permissible 8-hour time weighted average of 90 decibels (dBA). The equivalent dBA level was approximately 96.37 decibels (dBA). A sampling survey was conducted on March 24, 2015 for approximately 432 minutes. A zero time exposure was assumed for the period of time that was not sampled.     b) A grinder operator was exposed to continuous noise levels of 123.7 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). The equivalent dBA level was approximately 91.53 decibels (dBA). A sampling survey was conducted on March 24, 2015 for approximately 424 minutes. A zero time exposure was assumed for the period of time that was not sampled.      c) A injector molder operator was exposed to continuous noise levels of 169.5 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). The equivalent dBA level was approximately 87.3 decibels (dBA). A sampling survey was conducted on March 24, 2015 for approximately 448 minutes. A zero time exposure was assumed for the period of time that was not sampled.

29 CFR 1910.95(i)(4): The employer shall provide training in the use and care of all hearing protectors provided to employees.  The employer failed to provide training on hearing conservation, to include the use and care of hearing protectors, for all employees exposed to noise levels in excess of the action level and permissible exposure limit for noise. Employees were exposed to the hazard of hearing loss while performing assigned work activity involving the manufacture of plastic containers.   a) A injector molder operator was exposed to continuous noise levels of 69.5 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). The equivalent dBA level was approximately 87.3 decibels (dBA). A sampling survey was conducted on March 24, 2015 for approximately 448 minutes. A zero time exposure was assumed for the period of time that was not sampled.

29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:  a)Warehouse Area: The employer failed to ensure that a LP gas cylinder was secured on the Hyster-G-330892 powered industrial truck. The latch designed to secure the cylinder was broken. Employees were exposed to a fire/explosion hazard while operating the industrial truck to move containers and other materials in and around the warehouse area.
