29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. At the site, employees were exposed to a fall hazard, in that, they were spreading soil from a 11 foot roof with a 6/12 pitch, without fall protection, on or about 12/09/2015.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a. At the site, employees were exposed to a fall hazard, in that, no guard rail was in place while working from the 14 foot scaffold, on or about 12/09/2015.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.  a.  At the site employees, were exposed to a 14 foot fall hazard, in that, the scaffold was not fully planked while loading materials onto the roof, on or about 12/09/2015.   The  Company Lima Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(I), which was contained in OSHA inspection number 1050189, citation number 1 item number 2 and was affirmed as a final order on 06/30/2015 with respect to a workplace located at 19310 Gulf Blvd Indian Rocks Beach, FL 33785.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used cross braces were used as a means of access.  a. At the site, employees were exposed to a fall hazard, in that, the scaffold was used as a means of access to an 11 foot high roof, on or about 12/09/15  The  Company Lima Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(I), which was contained in OSHA inspection number 1050189, citation number 1 item number 2 and was affirmed as a final order on 06/30/2015 with respect to a workplace located at 19310 Gulf Blvd Indian Rocks Beach, FL 33785.
