29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  a. At the perimeter of the roof pop-ups, employees were exposed to a 60 feet fall hazard, in that, a single frame scaffold erected at the perimeter of the pop-ups with end frames placed at the building's edge was not provided with cross braces on both sides of the frames, on or about 04/08/2016.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a. At the roof of the  building around the pop-ups, an employee was exposed to a fall hazard when a ladder was not used for the purpose for which it was designed, in that, a six feet self-supported step ladder was used by leaning it against the wall of the pop-up and using it as a non-self-supported ladder, on or about 04/08/2016.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   a. At the roof of the building while installing exterior wall coverings, employees were exposed to a 60 feet fall hazard, in that, a single frame scaffold erected at the edge of the building was not provided with guardrails at the scaffold frame adjacent to the building edge, on or about 04/08/2016.    THE LIMA STUCCO, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(1)(vii) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1110869, CITATION NUMBER 01, ITEM NUMBER 01, ISSUED ON 02/02/2016 WITH RESPECT TO A WORKPLACE LOCATED AT 1706 W BEACH ST. TAMPA, FLORIDA 33606 AND BECAME A FINAL ORDER OF THE REVIEW COMMISSION ON 03/31/2016.
