29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a) On or about June 27, 2016, at the above addressed site, an employee was exposed to a fall hazard of approximately 20 to 22 feet when performing stucco work from the basket of aerial lift on the northwest corner of a structure while not wearing a safety harness with a lanyard attached to the basket or boom of that lift.  b) On or about June 27, 2016, at the above addressed site, an employee was exposed to a fall hazard of approximately 20 to 22 feet when performing stucco work from the basket of aerial lift on the southwest corner of a structure while not wearing a safety harness with a lanyard attached to the basket or boom of that lift.

29 CFR 1926.453(b)(2)(ix): Controls for the articulating and extensible boom platforms were not clearly marked to identify their function:  On or about June 27, 2016, at the above addressed site, an employee was exposed to fall hazard when operating an aerial lift which had the controls of that lift covered with stucco material and were not plainly marked as to their function.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about June 27, 2016, at the above addressed site, employees exposed to fall hazards while performing stucco work from aerial lifts were not provided a training program that would enable them to recognize the hazards of falling and those associated with the lifts.   PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
