At the site, employees were exposed to a struck by hazard in that, no head protection was worn while buckets fell and were thrown from a 20 foot 4 inch scaffold, on or about 01/16/2015.

At the site, employees doing stucco preparation work were exposed to a 20 foot, 4 inch fall hazard, in that, the scaffold was not supported by base plates or mud sills, on or about 01/16/2016.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph.  a. At the site, employees were exposed to a 20 foot, 4 inch fall hazard, in that parts of the scaffold had no top or mid rails. Cross members were used as fall protection and duct tape was used to hold the top rail in place on the third and fourth bucks, on or about 01/16/2016.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:  a. At the site, employees were exposed to a struck by hazard, in that the scaffold had no toe boards. Tools and equipment were falling from the upper levels of the scaffold, on or about 01/13/2015.

At the site, employees were exposed to a 20 foot, 4 inch fall hazard, in that the scaffold was not fully planked while applying soil to the roof, on or about 01/13/2016.  The employer, Lima Stucco, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1050189, citation number 1, item number 1 and was affirmed as a final order on 06/30/2015, with respect to a workplace located at 19310 Gulf Blvd Indian Rocks Beach, FL 33785.

Cross braces were used as a means of access.  a. At the site, employees were exposed to a 20 foot, 4 inch fall hazard, in that scaffolds members were used as a means of access and egress, on or about 01/13/2016  The employer, Lima Stucco, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number1050189, citation number 1, item number 2 and was affirmed as a final order on 06/30/2015, with respect to a workplace located at 19310 Gulf Blvd Indian Rocks Beach, FL 33785.  The employer, Lima Stucco, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1011707, citation number 1, item number 2 and was affirmed as a final order on 01/27/2015, with respect to a workplace located at 217 W. Belt Avenue, Bushnell, FL 33513.

At the site, employees were exposed to a 20 foot, 4 inch fall hazard, in that the scaffold was incomplete prior to being released and was not inspected by a competent person at the start of the work day and after moving the scaffold throughout the work day, on or about 01/16/2016.  The employer, Lima Stucco, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1011707, citation number 1, item number 3 and was affirmed as a final order on 01/27/2015, with respect to a workplace located at 217 W Belt Ave., Bushnell, FL 33513.
