29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On June 7, 2016, at the above addresses jobsite, employees were exposed to a fall hazard of about 19 feet while operating man lifts while performing stucco work without a means of fall protection. Two instances: a. Employee No. 1. Man lift JLG 400S # 96716; and b. Employee No. 2. Man lift JLG 400S # 947902.
