29 CFR 1910.304(a)(2): 29 CFR 1910.304(a)(2):  Grounded conductors were attached to terminals or leads so as to reverse designated polarity.    Plumbing Shop, Red Fir Facility, Sequoia National Park, 7/13/2016: Employees were exposed to electrical shock hazards when they worked with the RIDGID pipe threading machine which was energized by a double duplex receptacle that had reverse polarity.  The RIDGID pipe threading machine power cord was also missing the ground pin and the receptacle was not GFCI protected, which increased the probability of exposure.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    Plumbing Shop, Red Fir, Sequoia National Park, 7/13/2016:  Employees were exposed to electrical shock hazards when they worked with the RIDGID pipe threading machine which was missing its ground pin. The pipe threading machine was energized by a double duplex receptacle that had reverse polarity and was not GFCI protected, which increased the probability of exposure.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    Plumbing Shop, Red Fir Facility, Sequoia National Park, 7/12/2016: The 4 inch square duplex receptacle box, which was used to energize a tile cutter and a RIDGID pipe threading machine, had a 1 inch diameter unused opening that was not closed. Employees were exposed to electrical shock hazards if they were to come in contact with the live parts.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:     Red Fir Roadway Division Maintenance Facility, Sequoia National Park, 7/13/2016: Oxygen and acetylene cylinders were stored together which exposed employees to fire and combustion hazards.    The U.S. National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.253(b)(4)(iii), which was contained in OSHA inspection number 913208, citation number 02, item number 04 and was affirmed as a final order on 11/12/2013, with respect to a workplace located at 47050 Generals Highway, Three Rivers, CA 93271 (Ash Mountain Facility).

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.    Red Fir Maintenance Facility, Sequoia National Park, 07/13/2016: Two fire extinguishers were not mounted on the hanging brackets provided. The fire extinguishers were placed on the floor throughout the facility which exposed employees to tripping and fire hazards.
