Plumbing Shop, Red Fir Facility, Sequoia National Park, 7/13/2016: Employees were exposed to electrical shock hazards when they worked with the RIDGID pipe threading machine which was energized by a double duplex receptacle that had reverse polarity.  The RIDGID pipe threading machine power cord was also missing the ground pin and the receptacle was not GFCI protected, which increased the probability of exposure.

The U.S. National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.253(b)(4)(iii), which was contained in OSHA inspection number 913208, citation number 02, item number 04 and was affirmed as a final order on 11/12/2013, with respect to a workplace located at 47050 Generals Highway, Three Rivers, CA 93271 (Ash Mountain Facility).
