29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     a) On or about October 11, 2016, employees were exposed to the hazard of falling approximately 20 feet from the edge of the roof deck while engaged in roofing activities without the use of fall protection or a safety monitoring system.     No abatement certification or documentation required for this violation.

29 CFR 1926.502(f)(2)(ii): Warning line systems were not rigged and supported in such a way that its lowest point (including sag) was not less than 34 inches from the walking/working surface:        a) On or about October 11, 2016, employees were exposed to the hazard of falling approximately 20 feet where a warning line system along the roof edge was not rigged and supported to prevent sagging below 34 inches.     Solaris Roofing Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(f)(2)(ii) which was contained in OSHA inspection number 994715, citation 1, item 2b, and was affirmed as a final order on October 22, 2014, with respect to a worksite located at 851 Freehanville, Mount Prospect, Illinois.     No abatement certification or documentation required for this violation.

29 CFR 1926.502(f)(4):  Mechanical equipment on roofs was used or stored in areas where employees were not protected by a warning line system, guardrail system, or personal fall arrest system:       a) On or about October 11, 2016, employees were exposed to the hazard of falling approximately 20 feet while operating mechanical equipment at and along the roof edge without the benefit of fall protection.    Solaris Roofing Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(f)(4) which was contained in OSHA inspection number 994715, citation 1, item 3, and was affirmed as a final order on October 22, 2014, with respect to a worksite located at 851 Freehanville, Mount Prospect, Illinois.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
