29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and was not done in the following sequence: a. On or about May 15, 2017, employees were exposed to machine hazards associated with nip points when they engaged in servicing and maintenance activities on equipment located in the Can Assembly Department. The employer did not ensure that employees implemented energy control application steps during set-up activities, exposing them to amputation injuries from chain and sprocket assemblies on Line 2. The machines were not shut down or turned off prior to engaging in these servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147 (d)(4)], dissipation of residual energy [1910.147 (d)(5)(I)], and verification of isolation [1910.147 (d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or about May 15, 2017, in the Can Assembly Department, employees removing cans from Line 2 to be tested were exposed to amputation hazards from an unguarded conveyor drive chain. BWAY Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.219(f)(3), which was contained in OSHA inspection number 280194, citation number 1, item number 5 and was affirmed as a final order on 11/21/2012, with respect to a workplace located at 1202 Airport Rd. North Brunswick, NJ 08902.
