29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 07/26/17, on the west side of the house at 75 Como Ave., Buffalo, NY:  Employee was  exposed  to a potential fall of approx.  18 ft. from  the pik on a Ladder Jack Scaffold  while supplying materials such as flashing and ice shielding to other employees who were working on the roof.  No  Fall  protection system was being utilized.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:   a) On or about 07/26/17, on the west side of the house at 75 Como Ave., Buffalo, NY:  Employee was exposed  to a potential fall of approx.  18 ft. from  the aluminum  pik on a Ladder Jack Scaffold. There were 4 support ladders and 3 aluminum  pics.  Each end of the center aluminum  pic overlapped (bridged)  a different pic  by about 3 feet.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 07/26/17, on the west side of the roof on a house at 75 Como Ave., Buffalo, NY:  Employees engaged in roofing work on a steep roof were exposed to a fall of approx.  20 foot.  No fall protection system was being utilized.  Pitch of the roof was approx. 12 in 12.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) On or about 07/26/17, on the west side of the house at 75 Como Ave., Buffalo, NY:  Two employees working on the ground doing clean-up work in an area below employees working  on the roof and on a Ladder Jack Scaffold  were exposed to the potential hazard of being struck by materials/tools that could fall off the roof and scaffold. The employees were not wearing a hardhat.        NO ABATEMENT CERTIFICATION REQUIRED
