29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 07/08/19, on a roofing job site, at a house at 80 Delham Ave., Buffalo, NY: An employee was working from the aluminum piks of ladder jack scaffolding that was erected on the left side of the house. The employee was exposed to a fall of approx. 18 ft. because no fall protection was being utilized.  NO ABATEMENT DOCUMENTATION REQUIRED       Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1346658, citation number 1, item number 2 and was affirmed as a final order on 10/22/2018, with respect to a workplace located at 901 South Park Avenue, Buffalo, NY 14210.  Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1387205, citation number 2, item number 1 and was affirmed as a final order on 05/29/2019, with respect to a workplace located at 245 Lexington Avenue, Buffalo, NY 14212.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:  a) On or about 07/08/19, on a roofing job site, at a house at 80 Delham Ave., Buffalo, NY:  One employee of the crew was working from ladder jack scaffolding consisting of 3 support ladders, one approx. 20 ft. long aluminum pik, and two shorter approx. 12 ft. long aluminum piks, that were erected on the left side of the house. One of the short piks bridged the gap between the end of the long pik and the end of the other short pik. The piks were approx. 18 ft. above the ground.  NO ABATEMENT DOCUMENTATION REQUIRED         Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1302938, citation number 2, item number 1b and was affirmed as a final order on 10/18/2018, with respect to a workplace located at 446 Ashland Avenue, Buffalo, NY  14222.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 07/08/19, on a roofing job site, at a house at 80 Delham Ave., Buffalo, NY: Five employees engaged in roofing work on the steep roof on the left side of the house were exposed to a fall of approx. 20 ft. because no fall protection system was being utilized. Pitch of the roof was approx. 10 in 12.     NO ABATEMENT DOCUMENTATION REQUIRED
