29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 03/20/18, on a jobsite at a house on 446 Ashland Avenue, Buffalo, NY: Employees engaged in the installation of roof sheathing were exposed to a potential fall of approx. 19 ft. from the piks on a Ladder Jack Scaffold erected on the north side of the house. No Fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1250566, citation number 1, item number 1a and was affirmed as a final order on 09/19/2017, with respect to a workplace located at 75 Como Avenue, Buffalo, NY 14220.

NO ABATEMENT CERTIFICATION REQUIRED Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1250566, citation number 1, item number 1b and was affirmed as a final order on 09/19/2017, with respect to a workplace located at 75 Como Avenue, Buffalo, NY 14220.
