29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about 03/20/18, on a jobsite at a house on 446 Ashland Avenue, Buffalo, NY: Employees installing sheathing on   the  north side of the roof  were  exposed  to a potential fall of up to  21 ft.  because  no  fall protection system was being utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 03/20/18, on a jobsite at a house on 446 Ashland Avenue, Buffalo, NY: Employees  engaged in the installation of roof sheathing were exposed to a potential fall of approx. 19 ft.  from the piks on a Ladder Jack Scaffold erected on the north side of the house. No Fall protection system was being utilized.    NO ABATEMENT CERTIFICATION REQUIRED     Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1250566, citation number 1, item number 1a  and was affirmed as a final order on 09/19/2017, with respect to a workplace located at 75 Como Avenue, Buffalo, NY  14220.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:        a) On or about 03/20/18, on a jobsite at a house on 446 Ashland Avenue,  Buffalo, NY:  Employees were  exposed  to a potential fall of approx.  19 ft. from  the  aluminum piks on a Ladder Jack Scaffold  erected on the north side of the house because the aluminum  piks  were overlapped (bridged)  on the Ladder Jack Scaffolding .       NO ABATEMENT CERTIFICATION REQUIRED             Ackmen Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1250566, citation number 1, item number 1b and was affirmed as a final order on 09/19/2017, with respect to a workplace located at 75 Como Avenue, Buffalo, NY  14220.
