(b) Materials Center, Coater Line CO-6/4614: The employer failed to provide training to wash-up crews who were authorized to assist with roller change out operations and perform cleaning operations on machines, on or about 9/26/2019.    c) Materials Center, Coater Line CO-6/4614: The employer failed to provide maintenance employees with training on machine specific procedures, on or about 10/4/2019. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.     (a) Materials Center, Coater Line CO-6/4614: Employees were exposed to machine hazards associated moving machine parts and in-going nip points while they cleaned the top pan "catch tray" from excessive drips exposing them to unguarded, high speed rollers during the normal production mode of operation, on or about 9/26/2019.    (b) Materials Center, Coater Line CO-6/4614: Employees were exposed to machine hazards associated moving machine parts and in-going nip points while they cleaned outgoing belts which feed the conveyor which leads to the oven, on or about 9/26/2019.    (c) Materials Center, Coater Line CO-6/4614: Employees were exposed to machine hazards associated moving machine parts and in-going nip points while they changed out rollers on the machine, on or about 9/26/2019.     (d) Materials Center, Coater Line CO-6/4614: Employees had the potential to be exposed to machine hazards associated with moving machine parts and in-going nip points while they unjammed sheets of metal, on or about 10/4/2019.
