On or about February 22, 2022, with respect to an establishment located at 2121 Brooks Ave, Neenah, WI 54946, Neenah Enterprises, Inc. had not completed a periodic inspection of the energy control procedure for the Core Maker in Core Room 2 at least annually to ensure that the procedure and requirements of this standard were being followed.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). On or about January 25, 2022, with respect to an establishment located at 2121 Brooks Ave, Neenah, WI 54946, employees were exposed to machine hazards associated with moving parts when they engaged in a servicing activity for the "head" of the Palmer sand auger in Core Room 2. The employer did not ensure that employees implemented energy control application steps prior to allowing them to remove a lower chute for cleaning. An employee slipped in the area and inadvertently placed their hand in the opening of the auger resulting in partial amputations to four fingers of the left hand.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about January 25, 2022, with respect to an establishment located at 2121 Brooks Ave, Neenah, WI 54956, employees were exposed to potential machine hazards when: a) working with or near the Palmer sand auger head that was affixed with a chute to direct sand flow but did not provide adequate protection from the rotating auger. b) working or cleaning inside the Palmer Auger area of Core Room 2. Employees could access and area where the roll-over table would return to a home position and exposed employees to a potential struck by hazard. c) while working with or near the excess sand conveyor next to the Palmer Sand Auger operation area when in-running nip points near the top of the conveyor were not guarded to prevent access to the danger area.
