29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: in the areas identified in 1910.28(b)(1)(i)(A) - 1910.28(b)(1)(i)(C) (a) On or about May 15, 2022, the employer did not ensure employees accessing the Junker 2013 platform to perform rodding and skimming operations were protected from falling through the gap in the guardrails between the staircase and 2013 Junker platform, exposing employees to a fall hazard of approximately 13 feet 4 inches from the Junker platform. (b) On or about June 8, 2022, the employer did not ensure an employee engaged in scraping the channel on the Junker 2070 was protected from falling off an installed platform leading into the area, exposing the employee to a fall height of approximately 6 feet.

29 CFR 1910.29(b)(13): The employer did not ensure that when guardrail systems are used around holes that serve as points of access (such as ladderways), the guardrail system opening meets the requirements listed in 29 CFR 1910.29(b)(13)(i) or 29 CFR 1910.29(b)(13)(ii): a) The employer did not ensure that the guardrail where the ladderway was used for access on top of the CB30 machine was equipped with a self-closing gate or offset. b) The employer did not ensure that the guardrail where the ladderway was used for access to the mezzanine on top of the whiting office was equipped with a self-closing gate or offset.
