29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite located at the corner of 27th & Garfield, Kansas City, MO 64127: Employees were exposed to engulfment hazards while working inside an excavation at depths greater than five feet without any cave-in protection. Employees were tasked with replacing a water main. The depth of the excavation was approximately 6.6' to 7.1'.

29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At the jobsite located at the corner of 27th & Garfield, Kansas City, MO 64127: Employees were exposed to engulfment and fall hazards in that the ladder used for egress from the excavation was not set up to allow a safe means of egress. The ladder was positioned on a portion of the excavation approximately four feet higher than where the employees were working. Furthermore, the portion of the excavation where the ladder was positioned was made of loose gravel which did not provide a level and firm surface.
