29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite located at the corner of E 22nd and Grand Blvd, Kansas City, MO 64108: An employee was exposed to engulfment hazards while working inside an excavation at depths greater than five feet without any cave-in protection. The employee was tasked with cutting the bolt while replacing a water main. The depth of the excavation was approximately 7 feet 9 inches.
