29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At the jobsite located on Grand Blvd. between 22nd St. and Pershing Rd., employees were subjected to engulfment and tripping hazards due to inadequate means of egress from an excavation. The sole means of egress for the employees consisted of a ramp constructed from backfill and crushed rock. The excavation depth ranged from approximately 4.5 to 6 feet.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite located on Grand Blvd. in between 22nd St. and Pershing Rd., Kansas City, MO 64108: An employees were exposed to engulfment hazards while working inside an excavation at depths greater than five feet without any cave-in protection. The employees were tasked with connecting a pipe to a fitting. The depth of the excavation was approximately 6 feet 2 inches.
