29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At the job site located on the corner of Admiral Blvd. and Cherry St., Kansas City, MO 64106, employees were exposed to engulfment and tripping hazards due to inadequate means of egress from a 5'2" excavation. The lack of accessible ladders within the speed protective systems compromised workers' ability to exit swiftly in emergencies. This was observed in the following instances: 1. The ladder used for egress was placed outside of the Speed Shoring system. 2. The employee working in the trench box did not have a means of egress inside of the trench box.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the job site located on the corner of Admiral Blvd. and Cherry St., Kansas City, MO 64106, employees were exposed to engulfment hazards while working inside an excavation at depths greater than five feet without adequate cave-in protection. The employees were tasked with installing a water main. The employees were exposed to the hazards during the following instances: 1. Employee One was working at a depth of approximately 5 feet 2 inches inside a speed shoring system when they exited the trench support to access a ladder located outside of the protected area. 2. Employee Two was working at a depth of approximately 5 feet 1 inch inside of a trench box that was elevated approximately 4 feet 3 inches from the bottom of the excavation. Employee Two had to exit the protective system to exit the excavation.
