29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: At a worksite located at or near 2323 Grand Boulevard, Kansas City, MO 64108: Employees were exposed to struck by and crushing hazards. The employer failed to ensure that employees working with manufactured materials and equipment used for protective systems were not used in a manner that was consistent with the recommendations of the manufacturer. This was demonstrated on or about March 3, 2025, when employees were using a protective systems in an excavation by suspending the equipment over the employees who were working inside the excavation.
