The employer failed to ensure that employees working with manufactured materials and equipment used for protective systems were not used in a manner that was consistent with the recommendations of the manufacturer. This was demonstrated on or about March 3, 2025, when employees were using a protective systems in an excavation by suspending the equipment over the employees who were working inside the excavation.
