29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At a worksite located at or near 2345 Grand Boulevard, Kansas City, MO 64108: Employees were exposed to crushing or engulfment hazards while working inside an excavation. This was demonstrated on May 6, 2025 when employees were working at depths greater than five feet without any means of cave-in protection. The employees were tasked with connecting a pipe to a fitting where the depth of the excavation was approximately 6 feet 8 inches.
