What happened

This belly caused couplers to hang on the the trapeze as they advanced the pipe In the scenario involving the injured employee he initially attempted to reach overhead to free the piping He was out of reach of the piping so the employee climbed onto a nearby Scaffolding Pick Board The Pick Board would offer him more elevation to manipulate the piping The piping had been installed by forklifts and other lifting devices to reach the point at which it became hung The rigging attached to the piping developed some off center angle when the piping was hung While the employee was on the Scaffolding Pick Board the piping was freed causing the rigging to find the center of gravity The Employee was attempting to manually manipulate the piping when this unexpected movement occurred This resulted in a loss of balance and ultimately a fall The employees right shoulder made contact with the Pick Board
