Making fit on 4 inch pipe for welding activity at ground elevation

What happened

Two pipefitters were tasked with cutting and prepping a 4 inch pipe to be welded at ground elevation Each 20 feet long pipe joint was being supported by three jack stands While moving jack stands to make fit the employees manipulated the straight run by manually moving the jack stands During this process the pipe rolled out of one of the jack stand at the opposing end Once the pipe became free the second jack stand fell over from the momentum This caused the pipe to fall out of the third jack stand where the employee was standing The pipe made contact with the employees left foot causing him to trip on uneven surface The employee extended his left hand to brace his fall resulting in multiple fractures to the left wrist The initial impact to foot from the falling pipe resulted in a fracture to left foot
