On [REDACTED] TM stated that last [REDACTED] they lifted a heavy tote and their shoulder had been sore since. Employee initially filled out a Declination of Medical treatment form and self-administered first aid; on [REDACTED] she reported that the pain in her shoulder was still bothering her and she would like to seek medical treatment.
