Before the incident
On [REDACTED] around [REDACTED] TM was picking up a box of Baxter 's near [REDACTED] and felt a spike of pain in upper back that spread to shoulder blades. TM attempted to stretch it out then reported to Supervisor. TM stated it was still feeling very uncomfortable and hurt to lean left or take quick steps.TM taken to care room to sit and heat pack was applied. Heat did help but the level of pain was still high and TM wanted to get checked out by a doctor. The intensity of the original pain was scary and he did not want to leave it unattended. TM did not want to call Cor-Vel and was provided with heat and ice packs and some bio-freeze for self care at home. TM likely to go to Urgent Care and will update Supervisor with any treatment information.
